terminating its participation in the ACO REACH Model.
Benefit Enhancements/Beneficiary Engagement Incentives - Through its participation in the ACO REACH Model, CareMax Health Partners offered a Chronic Disease Management Reward (BEI) and a Skilled Nursing Facility (SNF) 3-Day Stay Waiver as Benefit Enhancements. Such services furnished under the Benefit Enhancement will no longer be covered by Medicare following a date that is 90 days after the effective date of termination, and Beneficiaries receiving such services may be responsible for the payment of such services. Beneficiaries receiving items and services or other remuneration pursuant to a Benefit Engagement Incentive should note such services will no longer be provided by the ACO following a date to be specified by CMS.
