In May, you received a letter or email explaining:

• Your provider is a part of an Accountable Care Organization, namely CareMax Accountable Care Network. This
  has no impact on your Medicare benefits.


• We have teamed up with your provider, hospitals, and other health care professionals to make sure you get the
  best coordinated care and stay well.


• You may have access to expanded benefits such as rehabilitation services or other skilled nursing facility care,
  without a hospital stay. You may ask your provider if you qualify for these benefits or you may contact us
  directly.


• Medicare protects the privacy of your health information. If you don't want Medicare to share information with
  your health care providers for care coordination, call 1-800-MEDICARE (1-800-633-4227).
